Providing skilled nursing visits for disease management teaching, wound care, Ostomy care, Medication teaching and management, coordination of patient care with all disciplines, home safety etc. At FSHC, field RN daily productivity is 6 points/day. Initial RN assessment is counted as 2 points and follow up visit is one point.
