Reporting to the Administrator, the Director of coordination will oversee the advancement, implementation, and management of coordination and care for our patients.
POSITION SUMMARY:
The Parent Care Director of coordination assumes responsibility for the coordination team and the service quality.
Responsible for care coordination to ensure proper communication between caregivers, patients, referral sources and MLTC's. Responsible for operational issues, hiring and managing staff.

Pay: $85,000.00 - $120,000.00 per year
