Bilingual Care Coordinator

We are seeking a Bilingual Spanish Registered nurse for a large Homecare agency to attend to patients in their home.

Responsibilities:

Coordinate, implement, and oversee care plan with patient, family, and support staff
DOH and Insurance compliance documentation for each visit
Clinical care includes IVs, wound care, catheters, trachs, etc.
Oversee case management
Assess the patient's medical needs and develop a plan of care with the physician
Qualifications:

Previous experience as an RN (Hospital, LTC, Home Care)
Bilingual Spanish (Required)
Home Care experience a Plus!
Current active Registered Nurse License
